The membership plan does not cover the following treatments and services:

Laser Hair Removal, Laser Skin Rejuvenation, Permanent Makeup & Microblading. Plan savings do not apply to certain services that are not offered at our establishment. In this case, you may be referred to a specialist who does not participate in our plan.

Disclosures

• Membership plan is not (i) insurance, (ii) qualified health plans under the Affordable Care Act, (iii) pre-payment for future services, or (iv) payment for access to discounted services, sales, or any other services than what is listed. Rather, Membership plans are direct payment arrangements between plan members and Unique Face Options for the services detailed and included in whichever plan the member selects.

• Payment for any additional treatments and/or services not included in the Membership plan is due at the time of service.The savings offered in the Membership plan can not be combined with any other advertised offers or discounts.

• Any lifetime plan member that fails to pay annual renewal fees automatically defaults to an inactive status. Inactive status will result in immediate suspension of exclusive membership benefits. Once the account is paid and therefore current, it is updated to reflect an active status and all member privileges are restored. There are no refunds. 

• Benefits, fees, and privileges may be revoked or amended at any time.
