The membership plan does not cover the following treatments and services:
Laser Hair Removal, Laser Skin Rejuvenation, Permanent Makeup & Microblading. Plan savings do not apply to certain services that are not offered at our establishment. In this case, you may be referred to a specialist who does not participate in our plan.
Disclosures
- Membership plans are not (i) insurance, (ii) qualified health plans under the Affordable Care Act, (iii) pre-payment for future services, or (iv) payment for access to discounted services, sales, or any other services than what is listed. Rather, Membership plans are direct payment arrangements between plan members and Unique Face Options, Inc. for the services detailed and included in whichever plan the member selects.
- Membership plans run for (12) consecutive months and include an auto renewal feature. Plan members can opt out of the membership at any time by calling or texting 404-735-7488. There are no refunds. Any payments processed prior to cancellation will become a credit on your account. The savings offered in the Membership plan can not be combined with any other advertised offers or discounts.
- If you have unused credit at the end of a month, they will continue to rollover and accumulate each month. Accumulated credit can be used towards facials. Membership credits will expire on your auto-renewal date.
- Benefits, fees, and privileges may be revoked or amended at any time.
